Team Leader (Rivercourt & Ashchurch)

Seven Social Care is looking for a Team Leader (Rivercourt & Ashchurch) for London Borough of Hammersmith and Fulham Council.

The role:

Varying shifts across the entire week.

Provide support to Residents attending the Borough’s Educational, Health and Well-being Centre.

Work with colleagues and people with learning disabilities in the promotion of Ashchurch Centre and its core values, whilst fulfilling and furthering the aims and objectives of the Service.

To deliver high quality Learning, and Leisure activities working within agreed policies, protocols, and guidelines specific to the Resident / person being supported.

Monitoring Projects in the community, Making sure that they are conducive to Residents needs. Working with Residents with Autism, and PMLD to live a full filling life.

To assist with the planning, organisation and delivery of appropriate activities for Residents  Including but not limited to Half Term and weekend activities for Residents

Residents Cafes, held during the week, and at weekends, where carers can attend with the person they care for and access support as well as sessions including relaxation, Dance, Photography, Arts/Graphics etc.

Deliver occasional information sessions for family carers/parent carers throughout the year aimed at helping families understand personalisation and a person centred approach for family carers of adults with Learning Disabilities. To deliver information on co-production, and the work we will deliver in Collaboration with professional services/organisations.

To deputise in the absence of the Service Manager, monitoring service provision to ensure all staff work to H&F quality assurance and national standards.

To support the manager with the implementation of an effective Local Action Planning cycle in response to strategic change and development.

To lead workstreams for the service as required under the guidance of the manager.
