Transition Social Worker

Seven Social Care are currently searching for an experienced and empathetic Transition Social Worker to join our team in Hammersmith, UK. The candidate will provide professional support and guidance for individuals transitioning through critical stages in their lives, including the elderly, the disabled, or individuals recovering from significant life events such as illnesses or loss.

Responsibilities:

1. Coordinate with patients, families, and healthcare providers to create a smooth transition plan for the clients.

2. Provide psychosocial counselling and support to individuals and their families to facilitate optimal adjustment during transitions.

3. Identify the needs and goals of individuals and come up with a concrete plan to meet these needs.

4. Conduct assessments and review plans to monitor the client’s progress.

5. Assure that client’s right to self-determination, confidentiality, and dignity are practised.

6. Coordinate a variety of services such as healthcare services, legal and financial assistance, housing, and other community support systems.

7. Maintain documentation and reports to keep track of the client’s progress, changes, and updated transition plans.

Qualifications:

1. Degree in Social Work or a relevant field.

2. A valid registration with the Social Work England.

3. Proven work experience as a Social Worker, preferably in a transition role.

4. Strong problem-solving abilities and excellent communication skills.

5. Ability to work independently and also as a part of a multidisciplinary team.

6. Knowledge of local community resources and services.

7. Excellent interpersonal skills and a passion for helping others.
