Transition Social Worker

Seven Social Care are seeking a dedicated and empathetic Transition Social Worker for a dynamic role in our Hammersmith location. You will be primarily responsible for guiding, supporting, and empowering individuals as they undergo major life changes. This role requires excellent interpersonal skills, as well as a deep understanding of social services policy and procedure.

Responsibilities:

1. Conduct comprehensive assessments of individuals needs preparing them for life transitions such as aging, disability, illness, or major life changes.

2. Collaborate with inter-professional teams and partner agencies to implement support plans that aid the transition process.

3. Advise individuals and their families on their rights and support available, working in partnership to enable informed decisions.

4. Monitor and evaluate the effectiveness of support plans and implement changes when necessary.

5. Providing counselling and emotional support to service users and their families.

6. Developing and maintaining professional relationships with other agencies and social services departments.

7. Produce concise and accurate records and reports as required.

8. Stay informed about developments in social work and the legal and policy framework which surrounds social care work.

Requirements:

1. A degree in social work and registration as a social worker with Social Work England.

2. A demonstrated history of working with individuals undergoing significant life transitions.

3. Knowledge of legislation, policies and procedures relating to transitioning social care.

4. Sound assessment, negotiating and organisational skills.

5. Excellent written and verbal communication.

6. Ability to work in a complex, fast-paced environment.

7. Experience in conflict resolution and crisis management.

8. Strong interpersonal skills, with the ability to work effectively with diverse groups of individuals.

9. Ability to maintain the confidentiality of sensitive information at all times.
