Transition Social Worker – (Adult Social Care)

Seven Social Care are looking for a dedicated and caring Transition Social Worker to join our Adult Social Care team in Hammersmith & Fulham. The candidate will work closely with young adults transitioning from Children’s Services to Adult Services and their families to ensure a smooth and effective transition.

Responsibilities:

1. Conduct detailed assessments of service users and their needs.

2. Plan and deliver support and individual care plans for young adults transitioning from children’s services.

3. Collaborate with a multi-disciplinary team of healthcare professionals to strategize and implement quality care plans.

4. Provide guidance and advice to service users and their families, helping them to make informed decisions.

5. Maintain accurate records and complete regular reports on service users’ progress.

6. Develop and maintain relationships with local providers and agencies to enhance services for service users.

7. Monitor and review the effectiveness of care plans continually, making any necessary adjustments.

8. Provide support during crisis situations, helping users effectively manage their issues.

9. Deliver appropriate information, advice and guidance around care and support services available.

Requirements:

1. Degree in Social Work.

2. Registration with the Social Work England.

3. Prior experience in a similar role dealing with transitions in care is desirable.

4. Strong communication and interpersonal skills.

5. Ability to work independently and as part of a team.

6. Comprehensive knowledge of the Care Act 2014 and the Children and Families Act 2014.

7. Solid understanding of safeguarding and protection issues related to vulnerable adults.

8. Excellent time management and organisational skills.

9. Flexibility to work outside typical office hours if necessary.

10. Ability to manage a varied and complex caseload effectively.
