Transition Social Worker – (Adult Social Care)

Seven Social Care is looking for a Transition Social Worker – (Adult Social Care) at London Borough of Hammersmith and Fulham Council

The role:

The post holder will play a key role in ensuring young people who may be eligible for Adult Social Care under the Care Act have an effective transition from Children’s services.

The role will include casework and life planning with young adults, their families and professionals. The postholders practice will be responsive, inclusive and community based.

They will work within the legislative frameworks (Children and Adults) and agreed Local Authority procedures. This will include taking appropriate safeguarding action as and when required.

To endorse and act in accordance with the principles of personalisation, ensuring that care and support are person centred and as far possible put the people with whom they work in control of their lives.

Tasks will include carrying out assessments of need, planning of services and reviewing outcomes with the individual, their personal networks and support providers.

As a social work professional based, you will bring significant expertise to complex cases in our specialist transition to adulthood team. The team is based with the adult learning disability service and works closely with the disabled children’s team and the SEN preparation for adulthood key workers.

To provide a professional social work service to adults with a learning disability, their families and carers who are ordinary residents of Hammersmith and Fulham in line with the requirements of the Care Act 2014.

To engage with individuals, carers and families to assess and intervene, and using professional judgement, employ a range of interventions promoting choice, control and independence.
